Who We Are

Bridge to Hope, Inc. is a non-profit agency that has worked tirelessly for over a decade in West Africa and the United States, to empower those who are disenfranchised due to their gender, ethnicity, or economic status. It’s focus is on housing and job placement, comprehensive case management, supportive and referral services experience, to the collaboration. The Bridge to Hope, Inc. Project Mainstream has provided temporary housing with meals and an array of supportive services to homeless and low-income, men, women, and families with children, including veterans and persons with mental illness.

 

Project Mainstream provides Interim/Bridge Supportive Housing services for homeless individuals with and without mental health needs and/or that have health conditions. The program consists of short-term housing in a safe, clean and stable environment, while accessing comprehensive case management, including basic medical services/health education, and other supportive services with meals, 24- hours supervision with lived experienced staff to ensure “Whole Person Care”. Care that will result in housing placement ready individuals.

Housing/Job search and placement; identification of appropriate housing; financial move-in and security deposit assistance; referrals to job-placement specialists; free on-site medical services; access to no-cost pharmaceuticals services; referrals to sober-living homes and access to numerous mental health social services facilities, assistance with public assistance benefit programs; budget and money management counseling; the provision of financial assistance to aid in the acquisition of an Identification card, birth certificates, drivers licenses, and/or other monies for housing and job placement.

 

The Bridge to Hope Case Management and Housing Locator Team, believe in “Whole Person Care”, thus, upon acceptance and entry into Bridge to Hope programs, the program participant meets with a Case Manager who identifies their unmet needs and develops an Individualized Service Plan (ISP) that clarifies the activities to employ to ensure that their needs are met.

 

Bridge to Hope staffs a Physician on-call, who provides basic medical wellness screenings, and health education programs, monthly. Additionally, Bridge to Hope has a licensed Medical Social Worker (MSW) on staff who works closely with the on-call Physician and Case Management Team, to better address and support the program delivery, the Case Management Team and the program participant.

Facilities

The program site consists of three separate connecting units, with 15 bedrooms and 9 bathrooms. The first unit houses the Case Management, Intake/Registration, Administration, and Security offices. The second unit is the home for Project Mainstream, and the third unit for Project Caring Arms Program Participants.

 

The facility is partially handicap accessible, and contains the following amenities:

  • Recreational areas (inside/outside)

  • Enclosed outdoor patio

  • Entertainment/television area

  • Washer and dryer units

  • Fully gated

  • Community Amenities, include close access to:

Eligibility

Entity pre-screened referrals from the Department of Mental Health, Department of Health Services, CES service providers, and other social service agencies., who are in need of housing for the homeless populations they serve.

Project Mainstream

Project Caring Arms

Project Caring Arms is a Post-Hospital Housing and Supportive Services solutions program, and provides Bridge Housing for homeless and displaced individuals, who are being discharged from hospitals and/or medical facilities, without a place of residence (home). The program will consist of short-term housing in a safe, clean and stable environment, while accessing housing and case management services, basic medical care, health education, meals, 24-hour supervision, and wellness, resulting in housing placement.

 

Services Provided:

  •  Transportation Available from Hospital (minimum fee)

  •  Housing in a safe, clean and stable environment 

  • Three meals and two snacks daily

  •  Weekly housekeeping services

  •  24-hour supervision and care

  •  Basic medical care

  •  Intensive case management

  •  Housing search and placement

  •  Supportive and referral services

  •  Access to Home Health Aid services

  •  Access to the following program offerings:

    •  Health Education

    •  Financial Literacy

    •  Job Employability Training

    •  Referrals to Vocation Education Programs

    •  Substance Abuse Treatment and/or Placement
